The issue is normally related to your phone line. However, try the following:

1) Check line to make sure it is working properly.
2) Check your connections to the phone line, cables, etc.
3) Send an AT&F&C1&D2 to the modem to ensure it reset to factory defaults.
ATZ restores an old profile.
4) Your modem may respond with NO DIALTONE if it is unable to connect
(though it should say NO CARRIER). Verify that it is dialing.
