The form has lines for: customer signature, name (printed), address, city,
state, zip code, telephone number, OG 100 serial number, store name, and
store location. The offer is valid from 25 Oct 96 to 31 Jan 97. That date
is probably the purchase date for the OG since the form says that all forms
must be redeemed by 28 Feb 97. (All you long time OG users may be out of
luck). Power pack includes PC connectivity kit, Cross Digital Writer Pen,
and 2 AA batteries. (The pen tip seemed too pointy for me, I ground it off to
match the point on the OG's built in pen. It also marks the WriteRight
screen protectors I'm using and may mark the OG screen. Be careful!) HP's
